Provide skilled physical therapy services and functional assessments to patients within their homes to improve mobility and independence. Responsible for completing federally required OASIS documentation to ensure regulatory compliance and support clinical outcomes.
Requires a Doctor of Physical Therapy (DPT) degree, current state licensure, and a valid driver's license with reliable transportation. Experience with home health care and OASIS documentation is strongly preferred.
A Home Health Physical Therapist provides skilled physical therapy services to patients in their homes, focusing on improving mobility, strength, balance, and functional independence. This role includes direct patient care as well as completion of federally required OASIS assessments to support clinical outcomes, care planning, and regulatory compliance.
To enhance patient independence and quality of life by delivering skilled physical therapy in the home setting while ensuring accurate OASIS documentation that supports high-quality, compliant, and outcome-driven care.
