Provide comprehensive pharmacy services, including prescription processing and patient counseling, for eligible beneficiaries at the Lawton Indian Hospital and satellite clinics. Specialize in intensive diabetes management using evidence-based guidelines and the interpretation of continuous glucose monitor data.
Must be a graduate of an accredited College of Pharmacy with a current, unrestricted US license. Requires either a PGY1 residency or board certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS).
Clinical Care & Patient Education Provide comprehensive, professional, and patient-centered pharmacy services for pediatric, adult, and geriatric patients, with a strong focus on diabetes management Deliver extensive patient counseling to ensure understanding of medications, side effects, drug interactions, and expected outcomes, with emphasis on diabetes self-management Evaluate glycemic trends using EHR tools and continuous glucose monitors (CGMs); interpret data to optimize therapy Manage and adjust medication regimens under collaborative practice agreements using evidence-based guidelines Support medication adherence by identifying and addressing barriers to appropriate medication-taking behaviors Perform CGM placement and provide education on use and lifestyle modifications, including basic nutrition guidance Medication Management & Pharmacy Operations Process, verify, dispense, and document prescription orders, ensuring medication safety and accuracy Manage refill authorizations, call-in prescriptions, and patient refill lines Oversee medication inventory, including controlled substances, vendor orders, and returns Maintain pharmacy automation systems and ensure proper stocking of medications and supplies Care Coordination, Compliance & Quality Improvement Utilize RPMS and the Electronic Health Record to document care and support continuity of treatment Assist with patient flow, scheduling coordination, and communication via phone and fax Maintain compliance with HIPAA, The Joint Commission (TJC), and safety/infection control standards Support quality improvement initiatives and participate in departmental and facility programs Maintain licensure, complete continuing education, and participate in required trainings and safety protocols
Graduate of accredited College of Pharmacy Permanent, current, full and unrestricted license to practice in the United States, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS) from the Board of Pharmacy specialties OR completion of a Post Graduate Year One residency.
