The Medical Coordinator is responsible for planning, tracking, and scheduling all annual, follow-up, and emergency medical appointments for clients. They must ensure consistent documentation and communicate effectively with medical staff and program directors to meet client needs.
Candidates must have a high school diploma or GED, with a bachelor's degree preferred, and a valid driver's license with a satisfactory record. Ability to remain calm in crisis situations and establish supportive relationships with individuals receiving supports is required.
Schedule all client medical appointments to state mandated licensing regulations Observe and monitor the client’s status through paperwork and follow-up appointments Observe and track clients' physical care including, but not limited to, annual and emergency appointments Provide accurate, timely, and consistent documentation of client medical appointments in the appropriate records Attend all pertinent medical appointments that need to be addressed more clearly and precisely to achieve the best results for the individual Communicate with supervisors, co-workers, and medical professionals’ regarding clients’ status Additional tasks and responsibilities may be assigned Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing Minimum Requirements High school diploma or GED; bachelor’s degree preferred Valid driver’s license with a satisfactory driving record Complete all state and agency required training per state guidelines Ability to establish a comfortable and supportive relationship with individuals receiving supports Ability to exercise good judgment and remain calm in crisis situations Excellent verbal and written communication skills Why Community Options?
