The Care Coordinator provides counseling, educational, and referral services to patients in an Opioid Treatment Program. They are responsible for conducting assessments, developing person-centered treatment plans, and providing crisis management and skills training.
A bachelor's degree is required, with specific licensure for social work degrees and a valid Indiana driver's license. Candidates must obtain an approved addiction counseling credential (such as CADAC, ICAC, or LACA) within one year of hire.
Job Requirements Bachelor’s degree required If incumbent holds a bachelor’s of social work degree, incumbent must also possess a current, unencumbered Licensed Bachelor’s Social Worker (LBSW) license in the State of Indiana Valid Indiana driver’s license and driving record that meets Eskenazi Health’s driving policy requirements
Within one year of the date of hire, incumbents must have one of the following FSSA/DMHA-approved credentials for providing counseling with an OTP:
