Provides recovery-oriented, in-home, and community-based services for adults with severe mental illness as part of an Assertive Community Treatment team. Responsibilities include case management, crisis intervention, and utilizing peer support tools to promote wellness and recovery.
Requires a High School Diploma or GED and a Certified Peer Support Specialist (PRS) certification in the State of Iowa. Candidates must be able to obtain CPI certification and complete specific peer support training within six months of hire.
Assures consumers receive clinically sound triage, referral, and ongoing behavioral health services by collecting information needed for accurate assessment and participating in treatment plan development. Provides mentoring, individual and group support, and teaches self-advocacy, motivation, and recovery-focused skills through ongoing engagement and feedback. Delivers assertive community outreach and sustained efforts to engage clients, including linkage to services such as psychiatric care, medication management, and representative payee support. Evaluates and monitors consumer treatment plans through follow-up assessments and recommends adjustments based on clinical findings and changing needs. Responds to crisis situations by providing intervention, recommendations, and appropriate support to stabilize and assist clients. Utilizes peer support tools such as WRAP, NAMI programs, and psychoeducation to promote wellness, recovery, and client understanding of illness, medications, and treatment. Maintains accurate and timely clinical documentation while demonstrating UnityPoint Health values, compliance standards, and collaboration with interdisciplinary teams.
Education: High School Diploma or GED required Experience: Ability to collect and assess client information and apply critical thinking/problem-solving Ability to work with clients, families, and community partners in a professional and sensitive manner Ability to manage multiple priorities in a fast-paced environment while maintaining composure Proficiency in Microsoft Office and EMR systems License(s)/Certification(s): Certified Peer Support Specialist (PRS) in the State of Iowa required Must complete Appalachian Consulting Group Peer Support Training Model within 6 months of hire CPI (Crisis Prevention Institute) certification required within 6 months of hire *Specific offers are determined by various factors, such as experience, skills, internal equity, and other business needs. The salary range listed does not include other forms of compensation which may include bonuses/incentive, differential pay, or other forms of compensation or benefits that may be applicable to this role.
