Range: $30.59/hr - $53.41/hr; pay is based on licensure level ESSENTIAL DUTIES AND RESPONSIBILITIES Completes psychosocial assessments and treatment plans in cooperation with patient and multidisciplinary team. Conducts a comprehensive assessment of the clinical needs of each patient prior to beginning individual, family or group therapy.Works collaboratively with the patient to develop a treatment plan addressing patient’s identified treatment goals. Delivers behavioral health care in accordance with patient evaluation, treatment plan and within the scope of their license. Works directly in treatment relationship with patients, individually or in groups, in consultation with Psychiatrist and or NP. Provides crisis assessment and intervention. Consults and coordinates care of patient with physician and or NP, therapists, and community agencies. Provides appropriate discharge planning. Refers patient or patient's family to other community resources. Participates in clinical collaboration and or psychiatry supervision to discuss cases and determine appropriate course of treatment. Maintains pertinent patient data and documentation in medical record in accordance with state, TJC and Mercyhealth system guidelines. Attends required meetings or staffing as indicated by Program Manager. Participates in performance improvement activities. Performs other duties as assigned. Complies with all Mercy Health System, State and JCAHO policies and regulations. EDUCATION AND/OR EXPERIENCE Master's degree in counseling, Social Work, Psychology or other related field CERTIFICATION/LICENSURE Must possess all required certification and licensure per the state in which they will be practicing.
