The Case Manager provides community-based support by assessing participant needs and coordinating individualized recovery and service plans. They connect individuals with essential resources such as housing, medical care, and behavioral health services while maintaining regulatory documentation.
A Bachelor's degree in Human Services or related field is preferred, along with two years of experience in behavioral health, addiction recovery, or disability services. Candidates must possess a valid driver's license and be able to obtain required background clearances.

The Case Manager provides community-based support and coordination of services for program participants. This role is responsible for assessing participant needs, connecting individuals with community resources, and supporting the implementation of individualized recovery or service plans. The Case Manager works collaboratively with multidisciplinary teams to promote recovery, stability, and independence for participants.
Bachelor’s Degree in Human Services or a related field preferred, or equivalent experience.
This position includes work in both office and community settings within the program service area and may require travel throughout the community.
