Collaborates with the care management team to support a multi-disciplinary approach to patient care by gathering documents and scheduling follow-ups. Manages referrals, verifies eligibility, and assists members with healthcare system navigation and resource coordination.
Requires a high school diploma or GED (LPN also accepted) and 2-4 years of experience in a healthcare setting. Proficiency in clinical data systems and strong communication skills are essential, with a degree in a health-related field preferred.

The care management support coordinator position is a non-Clinician role that works in collaboration with the care management team to support the multi-Disciplinary team approach of patient care. Care management coordination supports both members and the cm (Care management) team by providing resources, requesting, and gathering appropriate documents, scheduling members for care manager follow up, and completing other delegated tasks to support the cm team.
The care management support coordinator will work remotely within regions to cover needs across the state.
