Assess funding needs and determine eligibility for Medicaid Home and Community-Based Services (HCBS) Waivers for individuals with disabilities or elderly populations. Conduct Level of Care assessments, review medical documentation, and participate in appeal hearings.
Requires a bachelor's degree in human services, education, or health/medical fields, or an equivalent combination of education and experience. A valid driver's license and reliable transportation are mandatory.

Bachelor’s degree in: human services, education or health/medical; long-term care, gerontology, rehabilitation, health/disability case management, or children with special health care needs OR any equivalent combination of education and experience will be considered on a year for year basis.
Experience in healthcare, long-term care, disability services, or completing assessment. Familiarity with medical terminology and Medicaid waiver services.
