Coordinates pharmacy initiatives for high-risk patients and implements performance improvement projects to ensure safe, cost-efficient medication therapy. Collaborates with providers to impact quality measures such as CMS STARS, PQA, and HEDIS.
Requires a Bachelor's or Doctorate in Pharmacy with a Pennsylvania Registered Pharmacist license and at least 2 years of relevant experience or a PGY-1 residency. Preferred candidates possess BCPS or BCACP certifications and experience with insurance payer contracts.
Bachelor’s Degree Pharmacy or Doctorate Pharmacy. 2 years relevant pharmacist experience in patient care or population health or 1 year PGY-1 Residency. Skilled in critical appraisal of medical literature. Capable of independently reaching therapeutic decisions in patient care and communicating them to physicians and other involved health professionals. Ability to accurately monitor medications. Ability to make independent judgments under stressful situations at times. Identify, organize, and complete performance improvement projects related to medication management. Ability to work effectively in a team environment. Knowledge of laws, regulations, and accreditation standards related to medication management. RP - Registered Pharmacist - State of Pennsylvania Upon Hire Preferred Qualifications 5 years relevant pharmacist experience in patient care or population health or 1 year Residency PGY-1 or formal training in population health. Prior experience in working with health insurance payer contracts. Prior experience in Population Health. BCPS - Board Certified Pharmacotherapy Specialist - Board of Pharmacy Specialties Upon Hire or BCACP - Board Certified Ambulatory Clinical Pharmacist - Board of Pharmacy Specialties Upon Hire Physical Demands Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
