The Care Coordinator identifies community resources and develops individualized service plans for refugee children and families facing behavioral health crises. They maintain relationships with West-African community leaders and coordinate with multi-disciplinary teams to ensure client needs are met.
A bachelor's degree in Social Work or a related field is required along with at least two years of case management experience with children and families. Candidates must demonstrate an interest in community outreach and experience working with refugee populations.
