Responsibilities

Serve as a primary clinical resource for cardiology patients, providing assessment, triage, and care coordination. Evaluate patient symptoms and concerns via phone, patient portal, and other communication channels to determine appropriate next steps. Collaborate closely with cardiologists, advanced practice providers, and the care team to ensure timely, high-quality patient care. Coordinate diagnostic testing, procedures, referrals, and follow-up care for patients with cardiovascular conditions. Educate patients and families on cardiac diagnoses, medications, treatment plans, and heart-healthy lifestyle modifications. Review laboratory, imaging, and diagnostic test results and communicate provider recommendations to patients. Support patients managing chronic cardiovascular conditions such as heart failure, hypertension, arrhythmias, and coronary artery disease. Document patient interactions, assessments, and care plans accurately within the electronic health record (EHR). Prioritize and manage multiple patient needs in a fast-paced specialty clinic while delivering exceptional patient-centered care.

Qualifications

Current Registered Nurse (RN) license in the State of Iowa. Associate Degree in Nursing (ADN) required; Previous cardiology, specialty clinic, ambulatory care, critical care, telemetry, or medical-surgical nursing experience is great, but not required. Strong assessment, critical thinking, and clinical decision-making skills. Excellent communication and patient education abilities. Ability to prioritize multiple responsibilities and adapt in a fast-paced clinical environment. Experience with Epic a plus!
