The Physical Therapist is responsible for assessing, planning, and implementing individualized treatment programs for home health patients. They provide hands-on therapy and educate patients and families to improve mobility and restore function.
Candidates must be a graduate of an accredited School of Physical Therapy with a current state or compact license. A minimum of two years of clinical experience is required, with a preference for home health or geriatric backgrounds.
Description First Choice Home Health is seeking a Licensed Physical Therapist to join our team to providing PT services including assessment, treatment planning, implementation, and evaluation for our patients currently enrolled in our home health program in the Boise, ID and surrounding areas. Graduate of an accredited School of Physical Therapy. Current State license as a Physical Therapist, or a multi-state license issued by a state member of the National Physical Therapy Compact (PT Compact), if practicing in a state member of the PT Compact. Minimum of two years of experience as a Physical Therapist in a clinical care setting, with experience in Home Health, geriatrics, or medical-surgical preferred. Excellent interpersonal and communication skills. Strong nursing skills as defined by generally accepted standards of practice. Reliable transportation with valid and current auto liability insurance. Ability to work flexible hours and travel locally as needed. Ability to lift up to 50 lbs. and perform physical tasks associated with patient care.
