Key Responsibilities
Conduct behavioral health assessments and develop individualized treatment and transition plans.
Provide evidence-based interventions (CBT, DBT, MI, SBIRT, mindfulness, trauma‑informed care).
Manage acute behavioral health concerns, including crisis intervention and suicide risk evaluation.
Support behavioral health integration in primary care through collaboration, consultation, and care coordination.
Facilitate referrals to higher levels of care and connect patients to community and hospital resources.
Document all care accurately and timely while adhering to regulatory and compliance standards.
