Description Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have a strong background in patient care, with experience in various medical settings including hospitals, doctor's offices and ambulatory surgery centers. As an LPN, you will play a crucial role in providing high-quality nursing care to patients, ensuring their comfort and well-being while adhering to established medical protocols.
Provide direct patient care under the supervision of registered nurses and physicians. Administer medications and treatments as prescribed, including IV infusion and injections. Conduct patient assessments and monitor vital signs to evaluate patient health status. Assist with diagnostic evaluations and perform basic life support as needed. Maintain accurate medical documentation and records in compliance with HIPAA regulations. Collaborate with interdisciplinary teams to develop and implement individualized care plans. Educate patients and their families on health management, medication administration, and post-care instructions. Support patients with activities of daily living (ADLs) including bathing, grooming, and mobility assistance. Participate in discharge planning and case management for ongoing patient care needs.
Valid LPN license in the state of practice. Experience in critical care, emergency medicine, or acute care settings is preferred. Familiarity with electronic medical record (EMR) systems such as Epic, Cerner, or eClinicalWorks. Knowledge of ICD-9/ICD-10 coding and medical terminology is advantageous. Previous experience working with individuals with developmental disabilities or in memory care settings is a plus. Strong communication skills and the ability to work effectively within a team environment. Basic knowledge of anatomy, physiology, and infection control practices. Experience in pediatrics or geriatrics is desirable; familiarity with hospice or home care is beneficial.
