The WOC LPN provides nursing care for patients with wounds, drains, pressure sores, and incontinence under the direction of an RN. Responsibilities include collecting patient data, implementing therapeutic interventions, and providing patient education.
Candidates must hold a current State LPN License and a High School Diploma. Preferred qualifications include two years of experience specifically in wound care.
Job Summary Under the direction of the Registered Nurse, the WOC LPN provides nursing care to assigned patient populations. Organizes patient care and sets priorities for people with wounds, drains, pressure sores and incontinence. They will assist with the provision of care as established within the frameworks of Practice Acts. Minimum Job Qualifications Licensure or other certifications: Current State LPN License Educational Requirements: High School Diploma Minimum Experience: Other: Minimum of ten (10) hours of continuing nursing education annually in addition to the twenty (20) hours of mandatory licensure requirement Preferred Job Qualifications Preferred Licensure or other certifications: Preferred Educational Requirements: Preferred Experience: Two (2) years of experience. Wound Care experience.

Demonstrates knowledge and interest in Wound Care Meets all certification requirements defined by unit Demonstrates critical thinking skills Demonstrates sound clinical Skills and judgement Basic Computer skills necessary to operate computer systems on the unit Essential Tasks and Responsibilities Patient Assessment: LPN assists in the collection of data an assessment of the patients' needs and strengths. Assist with the collection of data that reflects the patient's current condition with focus on current wound care needs. Assist with the collection of data and reports findings to RN and other healthcare providers. Relevant data is documented according to hospital standards. Functions as a resource for wound care to the nursing staff Planning: LPN provides feedback to the Registered Nurse in the development of the plan of care for patients. The LPN assists with discharge planning.
The LPN carries out therapeutic interventions established for the wound care patient. Interventions are implemented with safety, skill, and efficiency and are adjusted according to patient response. Patient response is documented in the record. Provides patient with teaching and training from needs identified from the WOC RN. Documents teaching in the record.
The LPN provides feedback to the WOC RN in the evaluation of interventions and treatments and how they affected patient outcomes. Evaluates the patient response to nursing care and therapies. Documents response to nursing interventions according to hospital standards. Involves the patient, significant other and other team members in the evaluating the care of the wound patient.
The LPN participates in educational activities to improve clinical knowledge and enhance role performance. Participates in and completes annual competencies as indicated. Acquires a minimum of ten hours of continuing education in wound care in addition to mandatories a year.
