Acts as a liaison between patients, treatment teams, and community resources to facilitate safe transitions from inpatient psychiatric care. Responsibilities include developing treatment plans, securing insurance authorizations, and coordinating post-discharge appointments.
Requires a Bachelor's degree in behavioral science or a related field, though experience may be considered in lieu of a degree. Must possess a BLS certification and preferably have one year of experience in the mental health field.
We are hiring full time Discharge Planner to our join our Inpatient Team. This position is Monday – Friday, 8:00 am – 4:30 pm.
Wellfound Behavioral Health Hospital is a 120-bed inpatient psychiatric hospital located in Tacoma, Washington. We are Department of Health and Joint Commission accredited. We are an acute care psychiatric hospital providing care to both voluntary and involuntary patients.
• Basic Life Support (BLS) card through American Heart Association or American Red Cross required to start
• One year of related work experience in the mental health field strongly that demonstrates attainment of the requisite job knowledge skills/abilities
