The Pharmacist evaluates and monitors drug therapy for home care patients while collaborating with healthcare professionals to implement care plans. They are also responsible for compounding sterile mixtures and ensuring compliance with USP 797 and USP 800 standards.
Candidates must have a Bachelor of Science in Pharmacy and a valid Registered Pharmacist License in the state of practice. A minimum of two years of acute-care experience or one year of home infusion therapy experience is required.
Shift: Monday-Friday 9:00am-5:30 pm with flexibility
Evaluates and monitor drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals. Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice. Adheres to all appropriate standards of accreditationand participate in quality assurance activities. Coordinates and facilitate new referrals and admissions through collaboration with nurses, customer service and distribution. Reviews, receives and verifies all new patient orders and document on the respective order form. Assists the physician and other involved healthcare professionals with: Planning and implementing patient care in the home; Setting therapeutic objectives for drug therapy; Establishing parameters for monitoring the safety and efficacy of drug therapy; and Preparting regular progress reports. Conducts and document initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs. Monitors the outcomes of drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information. Provides consultation and answer inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use. Maintains thorough and appropriate documentation in the customer/patient record of all clinical activities and communication with patients, physicians and other healthcare professionals. Processes and check all medications, supplies and equipment prior to dispensing from the pharmacy. Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars and conferences Compounds sterile mixtures and non-mixtures, including, but not limited to parenteral nutrition, antibiotics, chemotherapy, hydration and narcotic analgesics, using aseptic technique where state laws permit Ensures compounding/clean room areas and related equipment are cleaned, sanitized and maintained in compliance with company policies and procedures, and USP 797 and USP 800 standards Documents all activities on appropriate logs. Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars and conferences Pharmacists’ duties are extended to include overseeing of the Pharmacy technician functions as required by Pharmacy Board rules
