Position Summary

Southern Highlands Community Mental Health Center (SHCMHC) is seeking a compassionate and motivated Case Manager/Care Coordinator Level 2 to assist individuals in accessing behavioral health, medical, social, educational, and community resources. This position works closely with individuals and families to coordinate services, reduce barriers to care, promote recovery, and improve overall health and wellness outcomes.

The Case Manager/Care Coordinator will collaborate with multidisciplinary treatment teams, community partners, healthcare providers, and support systems to ensure individuals receive comprehensive, person-centered care.

Essential responsibilities

Assess client needs and assist with service coordination.

Develop and monitor individualized service plans.

Connect individuals with community resources, healthcare services, housing assistance, transportation, educational opportunities, and other supports.

Advocate for clients and assist them in navigating complex systems of care.

Maintain accurate and timely documentation in accordance with agency, state, and federal requirements.

Participate in treatment team meetings and collaborate with internal and external providers.

Monitor client progress and assist with goal attainment.

Promote recovery-oriented, trauma-informed, and person-centered care.

Minimum Qualifications

Applicants must meet one of the following qualifications

  • Bachelor's or Master's degree in a human services-related field in accordance with West Virginia Board of Social Work requirements; OR
  • Licensed Psychologist (Master's or Doctoral degree from an accredited program); OR
  • Licensed Registered Nurse.

Preferred Qualifications

  • Experience in behavioral health, case management, care coordination, social services, healthcare, substance use treatment, or community-based services.
  • Strong communication, organizational, and problem-solving skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Knowledge of community resources and service systems.

Requirements

Candidates must hold a Bachelor's or Master's degree in a human services field, be a Licensed Psychologist, or be a Licensed Registered Nurse. Experience in behavioral health, social services, or substance use treatment is preferred.
