Provide community-based nursing services, medication management, and crisis intervention for individuals with serious mental health needs. Collaborate with a multidisciplinary team to support patient stability and independence in home and community settings.
Must hold a current Licensed Practical Nurse (LPN) license in West Virginia and a valid driver's license with reliable transportation. Experience in behavioral health is preferred along with strong communication and organizational skills.

Southern Highlands Community Mental Health Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT) team. This position provides community-based nursing services to individuals with serious mental health needs, helping them achieve stability, independence, and improved quality of life.
As a member of the ACT team, the LPN works collaboratively with the Registered Nurse and multidisciplinary treatment team to provide nursing support, medication management, health education, and crisis intervention services in home and community settings. The Licensed Practical Nurse (LPN) assists with daily medication dispensing and delivery, monitors medication compliance and effectiveness, and provides health education and support to consumers and their families. This position coordinates referrals to medical providers and community resources, assists with crisis intervention and support services, and maintains accurate documentation of services provided. The LPN works collaboratively as a member of the ACT multidisciplinary team, delivering community-based services in consumers' homes and other community settings while participating in a rotating monthly after-hours on-call schedule.
