The Case Manager provides education on the Medicaid waiver process and creates individualized support plans based on client needs and goals. They facilitate community-based team meetings and collaborate with service providers to ensure clients access necessary supports.
Applicants must have a Bachelor's Degree in Human Services and at least one year of professional experience working with individuals with intellectual and developmental disabilities. A valid driver's license, reliable transportation, and a designated home office setup are also required.

This position requires in-person meetings; therefore, applicants must live within the State of Indiana or within 10 miles of the state line.
• Case managers work with Individuals on all HCBS waivers.
