The SUD Case Manager provides comprehensive support and care coordination for clients facing substance use challenges. Responsibilities include developing individualized service plans and collaborating with multidisciplinary teams to promote client recovery and independence.
A high school diploma is required, while a degree in Human Services and CPR certification are preferred. Candidates must possess an NPI number/Ohio Medicaid ID and pass federal and state background checks.
The SUD Case Manager provides comprehensive support and coordination of care for clients facing substance use challenges. This role involves assessing client needs, developing individualized service plans, and collaborating with multidisciplinary teams to ensure the delivery of effective and client-centered services. The Case Manager is dedicated to facilitating clients' recovery and promoting their overall well-being and independence.
· Coordinate and provide care that is safe, timely, effective, efficient, and client-centered. · Manage case assignments, draft service plans, and review case progress regularly. · Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options. · Collaborate with therapists, clients, and client family members to ensure all client behavioral health needs are met. · Facilitate clients in achieving personal independence and assist in developing daily living skills. · Perform other duties as assigned. · Upholds Documentation and Billing practices associated with CARF, CMS, and as training indicates. Supervision Given: This position does not supervise any staff. Supervision Received: The SUD Case Manager reports to their assigned Substance Operations Managers.
· Strong interpersonal skills and the ability to communicate effectively with clients, families, and professionals. · Knowledge of behavioral health practices and community resources. · Ability to work independently and as part of a team.
· High school diploma or equivalent is required; · a college degree in Human Services is preferred.
· CPR certification is preferred. · NPI number/Ohio Medicaid ID is required (instructions on how to apply will be provided).
· Must pass all Federal and State background checks according to company policy.
