The Clinical Service Coordinator conducts comprehensive behavioral health assessments and develops individualized treatment and discharge plans for adults. They coordinate care with community partners and maintain clinical documentation while meeting monthly billable hour standards.
Candidates must hold a Master's degree in Social Work, Psychology, Counseling, or a related behavioral health field. They must be licensed or license-eligible in Georgia (LMSW/LCSW, APC/LPC, or LAMFT/LMFT) and actively pursue licensure upon hire.
The Clinical Service Coordinator is responsible for conducting comprehensive behavioral health assessments, treatment planning, care coordination, and discharge planning for adults receiving outpatient behavioral health services. Services may be provided in person or through telehealth platforms.
Conduct and monitor biopsychosocial and behavioral health assessments. Develop, implement, and monitor individualized treatment and discharge plans. Assess client appropriateness for outpatient groups and services, including referrals from: NGRI/IST programs Probation and parole agencies Regional referral sources Hospitals and internal programs Collaborate with treatment teams and group facilitators to develop effective interventions and treatment strategies. Coordinate client transitions and discharge planning to ensure continuity of care. Maintain accurate and timely clinical documentation, including updates to client records, releases of information, and treatment plans. Communicate with community partners, personal care homes, courts, probation/parole officers, treatment teams, and other stakeholders as needed. Prepare reports and participate in monthly NGRI/IST meetings. Attend Supported Employment meetings and provide updates regarding shared clients. Maintain a minimum productivity standard of 60 billable service hours per month. Connect clients and families with community resources including housing, financial assistance, legal aid, education, employment, and other recovery supports.
Master’s degree in social work, Psychology, Counseling, or a related Behavioral Health field. Eligible for Georgia licensure as one of the following: LMSW/LCSW, APC/LPC, LAMFT/LMFT Must actively pursue professional licensure upon hire.
