Manage the full nursing process for patients from admission to discharge, including assessment, planning, and evaluation. Coordinate care with a multidisciplinary team and provide supervision to LPNs and HHAs.
Must possess a current Illinois State RN License and CPR Certification. Previous nursing experience and familiarity with medical software and equipment are required.

Completing the nursing process in the delivery of patient care, which includes assessment, planning, implementation, and evaluation of patient outcomes. Total oversight of patient from admission to discharge. Developing a plan of care based on findings. Serving as patient’s advocate and maintaining ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care. Timely completion of all skilled nursing documentation. Providing direct supervision of nursing clinical care team members (LPN/LVNs & HHAs) Multidisciplinary team approach (coordinating with all disciplines of patient care, including PT, OT, ST, MSW, HHA's, and office staff)
