The QMHP provides mental health targeted case management and rehabilitative services to help clients improve functioning and achieve treatment goals. Responsibilities include conducting assessments, implementing skill-building strategies, and coordinating care with multidisciplinary teams in community settings.
A Bachelor's or Master's degree in a behavioral health or human services field is required. Candidates must demonstrate competency in MHTCM services and possess a valid driver's license and insured vehicle for travel.
The QMHP is responsible for providing Mental Health Targeted Case Management (MHTCM) and rehabilitative services, including skills training and intervention. This role supports clients in improving functioning, building coping strategies, and achieving treatment goals in home, school, community, and virtual settings.
Client Assessment & Intervention Deliver therapeutic interventions to enhance client functioning and interactions. Conduct CANS/ANSA assessments, develop and update individualized treatment plans. Provide structured, face-to-face sessions based on treatment goals. Implement skill-building and rehabilitative strategies to support daily living and self-management of symptoms. Service Coordination & Crisis Response Assist clients in identifying triggers, coping mechanisms, and social support. Provide crisis intervention services in line with agency protocols. Collaborate with psychiatrists, therapists, families, schools, and community resources. Ensure services are primarily community-based, with limited clinic-based sessions. Documentation & Compliance Complete progress notes, treatment goals, and related documentation within 48 hours of service delivery. Maintain accurate, timely records in compliance with State of Texas standards. Participate in clinical supervision and attest to completion of mandatory training. Team Collaboration & Program Participation Maintain a caseload depending on agency assignment. Attend monthly staff meetings, team consultations, and training sessions. Support continuity of care as an integral member of the clinical team.
Required: Bachelor’s or Master’s degree in psychology, social work, medicine, nursing, counseling, rehabilitation, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention. Demonstrated competency in provision and documentation of MHTCM and MH rehabilitative services.
Bilingual (English/Spanish). Prior experience with Medicaid documentation and mental health community programs.
Strong organizational, time-management, and documentation skills. Effective oral and written communication. Ability to work independently and as part of a multidisciplinary team. Conflict resolution skills and adaptability. Cultural humility and ability to work with diverse populations.
Flexible schedule; evenings and weekends as needed. Frequent travel to client homes, schools, and community locations required. Must have an insured vehicle, valid driver’s license, cell phone, and computer/laptop. Position may require sitting, standing, walking, and occasional lifting (up to 25 lbs). Ability to remain professional in high-stress and crisis situations.
