$65,000 - $80,000 / YEAR
Acts as a telephone liaison to coordinate incoming referrals and conduct intake screenings for prospective patients. Manages the initiation of care by documenting required information and communicating arrangements to families and healthcare providers.
Requires a graduate of an accredited school of nursing with a current Registered Nurse (RN) license in Michigan. At least one year of RN experience is required, with prior home health care experience preferred.
