Coordinate the transition of individuals from acute care settings into transitional housing while ensuring continuity of medical and behavioral health services. Provide engagement, screening, and wrap-around care coordination to promote long-term housing success and stabilization.
Requires a high school diploma or GED and at least one year of behavioral health case management experience. A bachelor's degree in human services or a related field and two years of experience are preferred.
The Project HEAL Case Managers will provide engagement, screening, care coordination, and linkage to ongoing treatment services through Claratel Behavioral Health and other community-based resources as needed.
Coordinate discharge planning with hospital or acute care providers and medical home/home health services for identified individuals Conduct transition planning to support movement from facility-based care into residential settings Facilitate enrollment and linkage to Claratel Behavioral outpatient services; including counseling, psychiatric services, and substance use treatment Coordinate care with residential providers, medical providers, behavioral health clinicians and community services agencies Assist participants with appointment scheduling, transportation planning, benefits linkage, and service navigations Monitor engagement in treatment and supportive services into housing Participate in multidisciplinary case coordination with Project HEAL partners Support continuity of care during the critical stabilization period following discharge Maintain documentation consistent with Claratel Behavioral Health Policies and applicable regulations Complete and maintain appropriate data entry into the HMIS (also known as the ClientTrack) System Other duties as directed by Program Manager and/or Director
High school diploma or GED At least one year of experience in behavioral health setting providing case management/community support services Must possess basic computer and typing skills. Must have care coordination skills to assist individuals with accessing/linkage to additional resources and services within the agency and/or community Must have transportation to and from work
Bachelor’s degree in related fields (such as human services, counseling, criminal justice, or social work) Two years of experience in a behavioral health setting providing case management/community support services NEEDED ATTRIBUTES Excellent interpersonal, problem solving and critical thinking abilities. Must be able to remain calm in crisis/mental health situations and respond appropriately. Always maintain confidentiality. Professional and effective communication with Community Partners, Stakeholders, Staff, Clients’ families/significant others and the Public.
