Requires at least two years of professional experience working with homeless populations and individuals with multiple diagnoses. An associate's degree in Social Work or Human Services and a valid driver's license are preferred.
Description Under the direct supervision of the Housing and Recovery Support Coordinator, Residential Case Manager will provide case management and crisis intervention services for the Areyto Apartments Supportive Housing services, sponsored by the Chemical Abuse Services Agency, Incorporated. The purpose of this program is to offer services that will prevent formally homeless and disabled individuals from becoming homeless again, to work with tenants to improve their quality of life, and to help create a positive residential community living environment. This will be a highly collaborative model of support services focusing on linking tenants to community mental health, substance use, medical resources, and developing on-site vocational programming. Thus, maintaining community relationships and working collaboratively is an integral element of this position.
