UT Health ET Home Health Physical Therapist (PT)

• The UT Health ET Home Health Physical Therapist plays a critical role in delivering high-quality, patient-centered physical therapy services within the home health care setting.
• This position focuses on assessing, planning, and implementing individualized treatment plans to improve patients' mobility, function, and overall quality of life.
• The therapist will work closely with patients recovering from illness, injury, or surgery, providing therapeutic interventions that promote independence and prevent further disability.
• Collaboration with interdisciplinary teams, including physicians, nurses, and social workers, is essential to ensure comprehensive care coordination.
• Ultimately, this role aims to support patients in achieving optimal physical health outcomes while maintaining safety and comfort in their home environments.

As a Physical Therapist, you will

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Applies concepts of infection control/universal precautions in performing patient care
• Assure continuity of quality patient care delivered with appropriate documentation
• Provide effective communication to patient/family, team members and other health care professionals
• Promote agency philosophy and administrative policies
• Maintain and complete all documentation within company stated guidelines.

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• 1 year of home health experience preferred
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance
• HomeCare HomeBase experience preferred

• Physical Demands
• Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
• Must be able to lift, carry, pull, or push up to 100 lbs..
