• Mentor Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs.
• This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes.

• We are looking for a reliable self-starter with a strong nursing background.
• Our RNs provide direct care and support overall patient health, making a real impact on the lives of those we serve.
• What You’ll Do Review and update patient medical records.
• Provide treatment under the direction of a physician.
• Assist patients with prescribed medication.
• Measure and record patient temperatures.
• Triage patients who are ill, injured, or experiencing behavioral issues, ensuring a safe environment for all.
• Document patient care activities accurately in medical records.
• Complete administrative tasks such as scheduling appointments and preparing reports.
• Keep medical records up-to-date and complete for each patient.
• Perform other duties as assigned by your supervisor.
• Education & Qualifications Current New Jersey RN licensure required.
• Ability to multi-task and meet deadlines.
• Effective communication skills to manage relationships with vendors and employees.
