Description Help Clients Build a Stronger Future Valley's CORE II program provides safe, evidence-based residential treatment for adults navigating mental health challenges, substance use disorders, trauma, and other complex life barriers. As a Case Manager, you'll play a vital role in helping clients achieve meaningful progress by providing rehabilitative services, coordinating care, supporting individualized goals, and building trusted relationships that foster lasting recovery. Job Summary & Deliverables Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met. Provides psychosocial rehabilitative services as described by the Medicaid manual Supports clients in following individualized care plans and identifying case management goals Completes documentation on services provided within Valley’s standards Meets productivity expectations as assigned by supervisor Distributes medications as necessary Transports clients to appointments as necessary
Education High School diploma or equivalent Licenses/Certification Case Manager certification (within 120 days) CPR certification (post hire as needed) Valley de-escalation certification (post hire) Driving positions require a minimum age of 21 and a current driver’s license
Previous social services or behavioral health experience Bachelor’s degree in Social Services, Psychology, or related field
