The Occupational Therapist evaluates patients and develops individualized plans of care to improve independence with activities of daily living and functional mobility. They provide skilled treatments and collaborate with a multidisciplinary team across skilled nursing and home health settings.
Candidates must be a graduate of an accredited Occupational Therapy program and hold a current state-specific license. Experience in skilled nursing or home health is preferred, though new graduates are welcome to apply.
The Occupational Therapist (OT) provides patient-centered occupational therapy services in Skilled Nursing, Long-Term Care, and Home Health settings. The OT evaluates patients, develops and oversees individualized plans of care, and supports patients in improving independence with activities of daily living (ADLs), functional mobility, and overall quality of life in accordance with regulatory and clinical best practices. Key Responsibilities Evaluate patients and develop individualized occupational therapy plans of care Provide skilled occupational therapy treatments focused on ADLs, IADLs, cognition, strength, coordination, and functional mobility Supervise and collaborate with Occupational Therapy Assistants (OTA/COTA) in accordance with state practice acts Monitor patient progress, modify plans of care as appropriate, and communicate changes to the interdisciplinary team Educate patients and caregivers on therapeutic techniques, safety strategies, adaptive equipment, and home modifications Complete timely, accurate, and compliant documentation in the electronic medical record (EMR) Ensure compliance with all federal, state, payer, and facility regulations, including HIPAA Maintain a safe treatment environment and adhere to infection control and safety protocols Demonstrate professionalism, compassion, and respect in all patient and family interactions QualificationsQualifications Graduate of an accredited Occupational Therapy program Current Occupational Therapist license in good standing (state-specific) Experience in skilled nursing, long-term care, or home health preferred (new graduates welcome to apply) Strong clinical, communication, and time-management skills Ability to work independently while contributing to a collaborative team environment Why Join Us Flexible part-time schedule Supportive leadership and collaborative care model Opportunity to work across multiple care settings Meaningful work making a direct impact on patient outcomes Diversity, Equity, and Inclusion are at the heart of Pivot.
