The RN Care Coordinator provides professional nursing care and coordinates continuity of care for oncology patients across multiple clinic locations. Key duties include performing health assessments, managing medication refills, triaging patient calls, and collaborating with interdisciplinary teams.
Candidates must be a graduate of an accredited professional nursing program with a valid RN state license and BLS certification. A minimum of five years of nursing experience is required, including at least one year of oncology experience.
