Provide assessment, case management, and crisis intervention for individuals with severe mental illness within a FACT team. Coordinate services between agencies and maintain compliant documentation while delivering care in-office and in the community.
Requires a Bachelor's degree in social work, psychology, or a related field, with a Master's degree preferred. Candidates must have 2-3 years of experience with chronically mentally ill populations and a valid NYS Driver's License.
Job definition: Work with the CN Guidance FACT team to deliver services to those individuals who are
diagnosed with severe mental illness and whose needs have not been met by more
traditional service delivery approaches.
A Bachelor's degree or higher in social work, psychology, rehabilitation counseling, or related field. Licensure with the NYS Education Department in a specific field is preferred.
Two years' experience working with the chronically mentally ill (Masters level) recommended. Three years' experience working with the chronically mentally ill (Bachelor's level) recommended. Competency in vocational rehabilitation must be demonstrated by at least one year of training and/or experience in integrated mental health and substance assessment and treatment or vocational assessment.
