The RN-Nurse Navigator provides individualized assistance and education to patients and families to overcome healthcare system barriers. They manage the care continuum for patients with complex chronic illnesses through assessment, planning, and implementation of care strategies.
Requires a BSN or Master's in Nursing, a valid California RN license, and BLS certification. A minimum of three years of RN experience is required, with a preference for experience in case or chronic disease management.
Education: Required: Bachelor of Science in Nursing (BSN) or Master’s degree in Nursing Licensure/Certification: Required: California Registered Nurse (RN) licensure; Basic Life Support (BLS) from American Heart Association (AHA); Additional certifications required as outlined based on assigned department: Advanced Cardiovascular Life Support (ACLS) from AHA: Upon hire: CC7581 EDCC Cardiology Clinic Neonatal Resuscitation Provider (NRP) certification from AHA and American Academy of Pediatrics (AAP): Upon hire: 6400 2N LDRP 6530 Neonatal ICU Level II Preferred: Certified in specialty practice area Experience: Required: Three (3) years of RN experience Preferred: Experience in chronic disease management, case management, utilization management, adult acute care or area of specialty Reports To: Director, Chief Administrative Officer, Service Line Administrator Supervises: N/A Ages of Patients: N/A Blood Borne Pathogens: Minimal/ No Potential Skills, Knowledge, Abilities: Ability to prioritize and develop collaborative relationships both internally and externally, Ability to work autonomously, Basic computer skills and knowledge of managed care, Knowledge and ability to function in a professional role and serve as a professional role model, Knowledge of current nursing, medical and health trends and practices in rendering of patient care, Strong organizational, interpersonal, written, verbal, teamwork, critical thinking skills, Working knowledge of insurance coverage and procedures and community resources Essential Responsibilities 1. Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations. 2.
Collect comprehensive data pertinent to the patient’s health or situation. 3.
Develop a plan of care that prescribes strategies and alternatives to attain expected outcomes across the care continuum for inpatient and/or outpatient procedures. 6.
Implement the identified plan. 7.
Interact with and contributes to the professional development of peers and colleagues. Participates on a team for data collection, health care outcome reporting, clinical audits, and programmatic evaluation. 12.
Collaborate with patient, family, and others in the conduct of nursing practice. 13.
Integrate ethical provisions in all areas of practice. Utilizes the ethic decision making model to drive issues and advocate for patient wishes. 14.
Provide leadership in the professional practice setting and the profession. 17. Practices within the scopes and standards of practice set forth by the American Nurses Association. 18. Practices within the scopes and standards set forth by professional organizations specific to specialty. 19. Participates in the development and ongoing success of certification and accreditation for the assigned program/department, as appropriate. 20.
