Requires 1-3 years of experience in case management with older adults or adults with disabilities and knowledge of housing and public benefits systems. A bachelor's or master's degree in social work or a related field is preferred.
Continuum of Care Resident Case Manager Department: Community Support Services Reports To: Manager of Direct Care Services FLSA Status: Full-Time, Non-Exempt FTE: 1.00 Pay: Expected: $29-$31/hr (Pay band: $29.81-$35.58/hr) Work Location: 55 & 95 Laguna Street offices, San Francisco, CA 94102 Work Schedule: Days of the week: Monday through Friday, 9:00 am – 5:30 pm, On-site presence required 5 days/week, some evening and weekend hours may be required.

The Continuum of Care (CoC) Resident Case Manager provides comprehensive, client-centered case management services to LGBTQIA2S++ older adults and adults with disabilities experiencing housing instability, medical concerns, and complex psychosocial challenges in the Openhouse and Mercy Housing 55 and 95 Laguna Street properties. This position operates under contract with the City and County of San Francisco Homelessness and Supported Housing department and must comply with all applicable federal, state, local, and contractual requirements governing publicly funded case management programs. The CoC Case Manager performs assessments, care planning, coordination, advocacy, documentation, reporting, and monitoring functions to ensure compliance with Openhouse, Mercy Housing, and CoC standards and to support client safety, dignity, and housing stability. ESSENTIAL FUNCTIONS The following duties are considered essential job functions under the California Fair Employment and Housing Act (FEHA) and the Americans with Disabilities Act (ADA):
1–3 years of experience working with older adults and/or adults with disabilities Demonstrated case management experience in community health, aging services, or homeless services systems Knowledge of housing and public benefits systems Strong documentation and organizational skills Ability to travel locally for home visits Fluency in verbal and written English
Bachelor’s or Master’s degree in social work, counseling, psychology, gerontology, or related field Experience with Medi-Cal, IHSS, Medicare systems, and SF Permanent Supported Housing Motivational interviewing or behavioral health experience Multilingual proficiency Sensitivity to and knowledge of the unique life experiences, challenges, and strengths of the communities supported by Openhouse’s mission. Experience working with older LGBTQIA2S++ adults or equivalent experience working with other populations with unique socialization and service needs including but not limited to dementia and Alzheimer’s disease, behavioral health, caregiver wellness, traumatic brain injury, etc. As a condition of employment: Criminal history background check (post-conditional offer) Motor Vehicle Record (MVR) review All background checks are conducted in compliance with: San Francisco Fair Chance Ordinance California Fair Chance Act Applicable federal, state, and local law PHYSICAL & WORK ENVIRONMENT REQUIREMENTS Ability to navigate residential environments and travel one block back and forth between them Ability to operate a computer and office equipment up to 8 hours per day Ability to work in a shared office environment Full COVID vaccination required (or approved accommodation).
