Supervise and provide bereavement services, including establishing plans of care and conducting home visits for patients and families. Lead grief support group sessions and maintain detailed documentation of bereavement services and risk assessments.
Requires a Bachelor's degree in Social Work, Psychology, or a related field, along with experience in grief and loss counseling. Must possess a current CPR certification, a valid driver's license, and access to a dependable vehicle.
