Job Status
Full Time, Non-Exempt SCHEDULE: Monday-Friday, 7:30am-3:30pm. Hours will fluctuate based on schedules and needs of the program.
Summary
- Provide direct care and skill-building support to individuals with disabilities, promoting independence, community integration, and achievement of individualized goals. This role focuses on implementing person-centered plans, supporting daily living skills, and ensuring a safe and engaging environment.
- Key
- Responsibilities: · Provide instruction and support to individuals in achieving personalized goals and objectives · Assist with activities of daily living including personal care, hygiene, and meal support · Promote socialization, communication, and participation in recreational and community activities · Support community integration, including outings and use of local resources · Encourage independence, choice-making, and self-direction · Maintain accurate daily documentation, including progress notes and incident reports · Assist in assessments and participate in care planning meetings · Transport individuals to activities, appointments, and program functions as needed · Maintain transportation logs, safety checks, and vehicle records · Respond to behavioral or crisis situations appropriately · Maintain financial and billing documentation, including

- Skills: · Strong interpersonal and communication skills · Ability to support individuals with diverse needs and abilities · Basic academic skills (reading, writing, and math) · Ability to follow individualized support plans and document progress · Strong organizational skills and attention to detail · Ability to work independently and as part of a team · Basic computer skills (word processing, spreadsheets, and data entry)

- Requirements: · Ability to lift and carry up to 50 pounds · Frequent bending, stooping, and physical assistance of individuals · Ability to safely transport individuals and operate agency vehicles · Ability to respond appropriately in emergency or crisis situations · Valid driver’s license and proof of vehicle insurance required

- Requirements: · CPR and First Aid certification · Preventing Disease Transmission (PDT) · Certified Restraint Training (CRT) · Medication Administration (Core A & B) · Completion of required trainings

Requirements
Education/Experience Requirements High School Diploma or equivalent preferred. A minimum of one year experience working with individuals with disabilities is required.
