Provides clinical care and patient management including triage, assessments, and routine laboratory work under provider supervision. Responsible for maintaining clinical records, preparing exam rooms, and ensuring patient safety and comfort.
Requirements summary
Requires a nursing degree (RN/LPN) or Medical Assistant diploma with corresponding state licensure or certification. Must possess the ability to perform phlebotomy and administer injections accurately.
Provides care within parameters of an RN license, LPN license or Medical Assisting diploma/certification including the following duties under the supervision of the health care provider:
Conducts assessment within guidelines established by the Nurse Practice Act or training of a medical assistant.
Directs patient management activities through triage in determining priority need for care and reports findings to provider.
Assists with the referral of patients to hospitals, health care specialists and enabling services; communicates patient health information to designated provider.
Performs appropriate care in assisting the provider in diagnosis and treatment.
Observes and corrects, if possible, environmental factors dealing with comfort and safety of patients.
Preforms all office clinical functions including the following: Prepares patient for examinations and procedures, answers patient questions, performs routine laboratory work, gives injections, preforms blood pressure checks, measures height and weight, checks vision.
Elicits and records medical histories maintains clinical records in accordance with facility policies and procedures into the center’s electronic health record.
Assesses and records applicable vital signs and clinical observations into the center’s electronic health record.
Prepares patient exam room and equipment for each appointment.
Provides treatment and assists with office procedures as ordered by the provider.
Orders medical and laboratory supplies; checks medical equipment, completes daily sign off log.
Performs office laboratory tests in compliance with CiLA regulations.
Prepares specimens to be sent to lab; maintains lab log.
Provides appropriate patient follow-up as ordered by the provider
Maintains cleanliness of all clinical areas, washes and sterilizes work areas and equipment.
Performs emergency care and CPR in an outpatient setting in accordance with facility protocol.
Records and reports data as required by various governmental agencies.
Participates in staff meetings/facility committees.
Supports the on-going performance improvement/risk management program in order to maintain and improve the quality of patient care provided by EVCHS.
Participates in continuing education and in-service programs.
Completes medication refills per EVCHS policy and procedures.
Completes prior authorizations as needed for referrals, medications, etc.
Orders DME supplies for patients under the order of provider.
Ensures timely rooming of patients.
Obtains consents needed for record of outside provider visits, labs, procedures, diagnostic, etc.
Site Work Location
Each employee has a primary work site(s) to which he/she is assigned. However, EVCHS has multiple sites. Therefore, any employee may be asked to work at another site if the need arises. Administrative employees and contracted service providers may work from their homes as agreed upon in their contract. No employee shall be asked to perform work outside his/her job description/capabilities.
Qualification
Education:
RN: Graduation from an accredited school of nursing, including any of the following programs: Associate Degree, diploma, or bachelor's degree.
LPN: Graduation from accredited Licensed Practical Nurse (LPN) program.
MA: Graduated from a comprehensive Medical Assisting program
Work
Experience: Educational experiences at a minimum.
License/Certification: Licensure in the State of Illinois as a registered professional nurse or licensed practical nurse or temporary license while awaiting Illinois license. CMA or RMA certification for medical assistant.
Skills: Requires ability to analyze and interpret a variety of patient signs and symptoms. Ability to provide patient care to patients with a variety of health care problems. Ability to successfully obtain blood samples via phlebotomy and capillary sticks. Ability to accurately prepare and administer patient medications/vaccines via intradermal, subcutaneous and intramuscular injections.

Work Environment
The performance of this job takes place in a health center setting. The employee will be exposed to moderate levels of noise in a moderately paced environment. There will be frequent patient or client contact over the telephone and in person. The employee will also work with other EVCHS departments to deliver optimal service and meet health center goals.
