Salary
$29 - $32 / HOUR
Provide skilled nursing care, including routine visits, medication management, and treatments to patients in their homes. Coordinate services and counsel patients and their families according to physician orders and care plans.
Requires an active Wisconsin LPN license, CPR certification, and a valid driver's license with insurance. Two years of nursing experience is required, with one year in home health preferred.

What does a Transitions at Home LPN do? Transitions At Home Licensed Practical/Vocational Nurse provide skilled nursing care to include, routine visits, medication management, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
