LICSW: $92,227 This Inpatient Psychiatric Social Work position will collaborate with other multi-disciplinary professionals on a 24 bed, adult inpatient psychiatric unit to evaluate patients' psychiatric, medical or physical condition and to assess client needs as part of the interdisciplinary team. Inpatient Social Workers work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community.
Responsibilities include:
Performing Individual psychosocial assessments
Conducting daily individual sessions with assigned patients and leading one or more weekly Social Work treatment groups,
Family outreach and counseling/psychoeducation as indicated
Coordination with outpatient treatment team if applicable
Timely completion of daily EMR documentation, psychosocial Assessments, Treatment Plans and Discharge Summaries that will satisfy clinical and regulatory requirements and arranging referrals related to aftercare as needed.
Collaborates with and provides social work consultation to other disciplines within the setting. Participates in daily Patient Rounds and Team meetings
