The provider delivers nursing and medical services to individuals and groups within post-acute facilities, focusing on health promotion and disease prevention. They work autonomously and collaboratively to diagnose and manage acute and chronic health problems.
Candidates must hold current Florida licensure as an APRN (Adult Gero or Family) or PA, along with AANP or ANCC certification. Experience in skilled nursing facilities, sub-acute care, or geriatrics is preferred.
