The LPN is responsible for providing direct care to adult patients in an outpatient setting, including administering medications and vaccines. They collaborate with a multidisciplinary team to implement care plans and maintain patient documentation via EMR.
Candidates should be self-starters who enjoy collaboration and working in a team-oriented environment. At least 6 months of direct nursing care experience is preferred but not required.
GENERAL PURPOSE: In an outpatient setting, remains accountable for provision of direct care to assigned patients. Contributes to unit activities and supports enhancement of the care delivery system. Works as a team member and assists other staff members with implementing patient care. Assumes responsibility of care for assigned patients under the direction of the co-assigned RN or MD. Participates in the plan of care, implements appropriate nursing interventions, and contributes to patient/family education through collection of data, interdisciplinary team collaboration, documentation and excellent communication. Responsibilities include direct patient care, administration of medication/vaccines, minor laboratory testing, communication with patients and providers using EMR.
