Job description

Responsibilities: \n\nThe Home Health Aide is responsible for performing health related tasks and may\nalso provide assistance with personal hygiene, housekeeping and other related\nsupportive tasks to a client in his/her home or facility under the supervision\nof a registered nurse. \n\n \n\n * Carries out the agency’s mission, philosophy, goals and objectives within\n guidelines of Agency policy and position function. \n * Participates in the development and implementation of a realistic\n goal-oriented plan of care. \n * Provides personal care and rehabilitative services and assists in activities\n of daily living as directed by the Director of Clinical Services or the\n supervising Registered Nurse. \n * Follows care plan instructions as directed by Registered Nurse. \n\n \n\nQualifications: \n\n * Current NYS DOH approved Basic Training in Home Health Aide OR Equivalency\n Exam or Competency Only Program in Home Health Aide Services
