Physical Therapist Assistant - Float PRN

Physical Therapist Assistant | PRN Float | Nashville, TN All Heart Home Care is seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our home health team as a PRN Float in the Nashville area. This flexible, as-needed role offers the opportunity to provide personalized, one-on-one care to patients in the comfort of their homes while supporting multiple locations as needed. Key Responsibilities: Provide in-home physical therapy services under the direction and supervision of a licensed Physical Therapist Assist in implementing individualized treatment plans to improve patient mobility, strength, balance, and overall function Monitor patient progress and report any changes to the supervising Physical Therapist Educate patients and caregivers on prescribed exercises, safety protocols, and home care techniques Maintain accurate and timely clinical documentation in compliance with company policies and regulatory requirements Collaborate with the interdisciplinary team to ensure coordinated and effective patient care Travel to patient homes throughout the Nashville service area as needed

Qualifications

Graduate of an accredited Physical Therapist Assistant program Active Physical Therapist Assistant license in the state of Tennessee Current CPR certification Valid driver's license, reliable transportation, and current auto insurance Strong communication and interpersonal skills Ability to work independently and adapt to varying patient needs and locations Previous home health experience preferred but not required Work Schedule: PRN (as needed) Flexible scheduling based on patient needs and coverage demands About All Heart Home Care: At All Heart Home Care, we are committed to delivering compassionate, high-quality care to patients in their homes.
