Evaluate patients and create comprehensive care plans while coordinating with families and the care team. Serve as the primary driver of the care team to ensure high-quality hospice and palliative care.
Requires a nursing degree from an accredited school, a current RN state license, and at least 2 years of clinical nursing experience. Must have reliable transportation and the physical ability to lift 80-100 lbs.
A heart to serve patients and families and a passion for providing the best possible care Education: Graduate of an accredited school of nursing with a current state license as a registered nurse Experience: 2+ years of nursing experience in a clinical care setting (hospice experience preferred) Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively
