The Case Manager identifies and enrolls eligible veterans into services to ensure access to safe, permanent housing and essential support. They develop Housing Stability Plans and coordinate with community resources to promote long-term self-sufficiency.
A bachelor's degree is preferred, though a high school diploma is required along with at least 2 years of case management or social services experience. Candidates must possess a valid driver's license and proficiency in Excel for reporting and data management.
