The Clinical Coordinator manages OB tracking from pregnancy confirmation through postpartum support, ensuring high-quality clinical outcomes. Responsibilities include coordinating patient care, facilitating high-risk meetings, and linking patients to social support services.
Requires a high school diploma, graduation from an approved practical nursing program, and an active LPN license. Candidates must have 1-2 years of OB/GYN experience and current AHA CPR/BLS certification.
The primary responsibility of a Clinical Coordinator OB/GYN is to maintain a high degree of excellence in clinical outcomes while providing effective coordination of patient care. Responsible for carrying out the primary functions of OB tracking, which begins when the pregnancy is confirmed and continues through the first six weeks of postpartum support. The Clinical Coordinator OB/GYN will participate in daily huddles, high-risk meetings, identify the patient's needs according to protocols, and provide point of care services and education. This position will work closely with the patient and clinical team members to ensure that all clinical measures are addressed in a timely manner. The Clinical OB/GYN Coordinator will make each patient their priority when providing services and assisting them to meet their individual needs.
Education & Experience: High school diploma or equivalency diploma (GED) required. Graduation from your approved practical nursing program required. Minimum of 1-2 Years-Previous OB/GYN experience required. Certification/ Licensure: LPN required. Active AHA Healthcare Provider CPR/ BLS required. Knowledge & Skills: Must have critical thinking skills and maintain confidentiality. Excellent organizational skills and ability to handle a variety of tasks simultaneously. Knowledge of medical terminology and/or behavioral health topics specifically in obstetrics. Strong decision making and prioritization skills. Ability to work respectfully and professionally with the community, patients, families, and staff. Sympathetic, mature, responsible, and reliable. Strong patient engagement, interpersonal, and communication skills and ability to establish a therapeutic relationship with the patient. Position Responsibilities Perform OB intake and document past medical history. Track and input Prenatal Risk Assessment forms and perform any needed follow up. Assess needs from EPDS forms and track completion along with any needed follow up. Facilitate MFM/ATU appointments. Work through lab reporting errors or difficulties. Creates New OB bags and prepares items for patients. Assist patients with management of their pregnancy and links patients to social support services. Manage the referral process for regular and specialty referrals from scheduling, tracking, and ensuring patient attendance. Order's and tracks appropriate lab/radiology studies. Send letters and perform follow-up phone calls to patients for planned visits. Advocate for patient services with community, social service, and medical providers. Connect patients to Inspira and non-Inspira resources as appropriate and track all resources available to patients. These services may include but are not limited to preventive health services, behavioral health, dental, and care management. Track patient's adherence with plan of care in electronic and communicate outcomes to providers. Prepares monthly reports for provider meetings and monthly updated reports for high-risk meetings. Assisting provider with reviewing patient charts, patient follow-up Requests and tracks patients record from previous health care facilities. Ensures OB Database is updated on a regular basis when in knowledge of deliveries, transfer of care, miscarriages/terminations of pregnancy, changes to EDD and changes of Provider. Reports positive STI/STD findings to the state. Meets with pharmaceutical representatives, logs medications, and consistently organizes medication closet. The job involves working with patients, physicians, and support staff to meet patient goals successfully.
