• In this role, you’ll support residents across assisted living, memory support, and skilled nursing, assessing, planning, and providing high-quality care while partnering closely with residents, families, and an interdisciplinary team to develop and implement individualized care plans.
• You’ll play a hands-on role in supporting daily needs, documenting care with accuracy, and communicating openly to ensure continuity and trust.
• As a leader on the floor, you’ll guide and collaborate with CNAs, respond confidently in emergency situations, and uphold all federal, state, and organizational standards of care and confidentiality.
• We’re looking for someone who is not only clinically skilled, but also committed to teamwork, continuous improvement, and creating meaningful moments for those we serve.
• A current LPN license and CPR/AED certification are required, along with a passion for delivering safe, respectful, and engaging care.
• PRN position available.
• Flexible schedules, weekend, and holiday availability required.
• QUALIFICATIONS AND SKILLS Required: Current State Nursing License.
