Requires a degree in Occupational Therapy and an active, unencumbered OT license in South Carolina. Experience in skilled nursing or rehab settings and familiarity with PointClickCare and Casamba are preferred.

As an Occupational Therapist (OTR), you will evaluate and treat residents to improve their ability to perform daily activities, restore functional independence, and enhance overall quality of life. You will collaborate closely with physicians, families, and the interdisciplinary team to deliver personalized care. Key Responsibilities Perform patient evaluations and develop individualized treatment plans Provide therapy focused on improving ADLs, strength, coordination, and functional independence Monitor patient progress and adjust treatment plans as needed Maintain accurate and timely clinical documentation Communicate with physicians, residents, families, and care team members Participate in care planning and discharge planning Conduct staff education and in-service training Qualifications Degree in Occupational Therapy (Bachelor’s required; Master’s or Doctorate preferred) Active, unencumbered OT license in South Carolina (required) Must meet all continuing education requirements and remain in good standing with the State Board Experience in a skilled nursing or rehab setting preferred Familiarity with PointClickCare (PCC) and Casamba preferred Strong clinical, communication, and organizational skills Why Join Fleetwood Post Acute?
